This questionnaire has been prepared by Petersen Legal to assist with completing the standard intake processes for parenting mediations (properly called Family Dispute Resolution Conferences). It provides the completed information to our office. It does not file the information with outside parties.

• The questions on this assistant will take 10-15 minutes to complete.

• You cannot save your answers before completing, or alter your answers after submitting, to protect your privacy.

• Once we receive your answers, we will use them to prepare an initial report to the mediator. It is not our standard process to disclose this to the other party.

• No information is submitted until you hit the Submit button.​ All responses are password protected once lodged.
